Traumatic Brain Injury (TBI) results from external mechanical forces causing structural or functional brain damage. Cases span mild concussions to severe injuries with prolonged unconsciousness or amnesia — and even "mild" concussions can have lasting neurological consequences, especially with repeated injuries.
Injuries to different brain regions produce different symptoms — and different biomarker signatures. GFAP rises when astrocytes are damaged. UCH-L1 and NfL rise when neurons and axons tear. A multi-marker panel reads the pattern across regions rather than testing a single signal in isolation.
Calcium-binding protein released from damaged astrocytes. Rapidly elevated after brain injury.
Clinical use: Early screening tool in emergency departments. High negative predictive value can help rule out the need for CT in mild TBI.
Glial Fibrillary Acidic Protein — structural protein from astrocytes. FDA-cleared biomarker for identifying intracranial lesions on CT.

Ubiquitin C-Terminal Hydrolase-L1 — neuronal protein abundantly expressed in the brain.
Clinical use: FDA-cleared in combination with GFAP for TBI evaluation. Indicates neuronal damage severity.
Neurofilament Light — structural component of the neuronal cytoskeleton. Indicates axonal damage and neurodegeneration.
Clinical use: Prognostic marker for long-term outcomes. Elevated in the chronic phase indicates ongoing neurodegeneration.
Microtubule-associated protein. Phosphorylated tau (p-tau) indicates neuronal injury and potential for chronic neurodegeneration.
Clinical use: Predicts risk of CTE and post-traumatic dementia in repeated concussion cases.
Neuron-Specific Enolase — glycolytic enzyme from neurons. Elevated in moderate-to-severe TBI.
Clinical use: Prognostic indicator for neurological outcome and mortality in severe TBI.

Elevated GFAP with borderline UCH-L1 is consistent with patterns observed in acute astroglial activation accompanied by early neuronal involvement. NfL and Tau remain within reference range.
